PKHD Consent Form: Version 6 Published

PKHD LogoFollowing the consultation process, which ended on 19th October, NAFIS have received a number of suggestions from FIS which we have used to inform final revisions to Version 6 of the PKHD Consent Template. Can we take this opportunity to thank you all for your contributions during the consultation. Below you will find a link to the final version of the template. As before this is a suggested template which you are under no obligation to use and we would suggest that you have the content approved by your own legal advisers. NAFIS believes that the template covers all the necessary legal points that need to be addressed before uploading providers’ data to PKHD and other channels.

Please note- You will need to consult with your own legal advisors and data protection officers to satisfy yourself that by using this form you will be compliant with data protection legislation. (As a ‘Data Controller’ under the Data Protection Act 1998 you need to have developed your own procedures and documentation regarding the handling of this personal data. Your attention is drawn to sections 7.10 and 7.11 of the statutory guidance on the Section 12 duty – Duty to provide information, advice and assistance: Guidance for local authorities)

You also need to satisfy yourselves that by using the form you will be able to fulfil your obligations under v1.02 of the PKHD terms and conditions. Whilst we have reviewed this template in the light of the latest terms and conditions, it is ultimately up to you to satisfy yourselves that this is the case.
